Agreement Between Carrier And Shipper
Agreement made (agreement Date), Between Mr. Barry Peterson (USA Hauler), a corporation organized and existing
under the laws of Pennsylvania, with its pricipal office located at 106 Sunset Ave, Hanover, Pa. 17331. Referred to
in this agreement as carrier and (Your Name )

In consideration of the mutual covenants and promises set fourth in this agreement, carrier and shipper agree
as follows:
Transportation of Goods
For the consideration discribed in the agreement, carrier shall ship the following described goods from
shipper

Time of Pickup and Delivery

Shipper's preferred arrival date is
. Although carrier transports all shipments with reasonable dispatch, carrier cannot operate on a set schedule.
If delivery cannot be made within the period set forth, however, carrier will make every effort possible to notify shipper
when delivery can be expected. It is the shipper's resposibility to make sure someone is present at time of delivery
to recieve goods. There will be a charge of $20.00 a hour if shipper or someone acting as shipper is not present after
the first 1/2 hour of the carriers arrival unless other arrangements has been made. Carrier will not release and goods
to persons acting on shipper's behalf without any photo identification (driver's license, Military I.D. etc). Shipper
must notify carrier of person/s recieving tranported goods or carrier will not release goods for you protection.
Storage Service
Storage may be ordered by shipper at any time from pickup to delivery, except where the storage service is
in transit, carrier shall issue its standard warehouse receipt. That receipt shall then supersede this agreement
upon acceptance of terms made by both parties. A charge will be made for warehouse labor if needed wrapping, packing,
and accessorial service is warranted. Storage is authorized in any warehouse of carrier choice upon agreement of storage
terms and conditions. Shipper authorizes carrier, upon agreement, to place the goods in storage at destination in the
event delivery cannot be reasonably made upon arrival. Shipper further authorizes the advancing of any additional charges
for storage, handling, and transportation that accrue if the goods are not reasonably accepted at destination.
Limitation of Liability
The released value of goods to be transported is hereby specifically stated by shipper to be (
$ ), total for the article/s. The rates quoted in this agreement
are based on such agreed or released value, and carrier' liability is limited accordingly. Protection against
loss or damage exceeding the carriers liability under this agreement maybe secured, if desired, by obtaining additional insurance
coverage through your insurance agent or broker. Absolutely no personal items, such as loose gear, lines, life jackets,
marine electronics, car stereo equipment, cb, vhf radios, tack, etc. are covered by carrier. Carrier will reasonably
inspect goods before transportation and notify shipper if any goods need to be secured or removed for shipment. If shipper
makes any attempt to use covers or tarps on any vehicle, boat or trailer, it is shippers full responsibility to make sure
covers/tarps do not interfere with carriers line of sight (flapping in the breeze while in transit). If carrier deems
it to be a hazzard to line of vision, carrier will make every attempt to secure such cover device so that it may not
be a hazzard or contribute to adverse driving conditions. Should damage result due to improper mounting of tarps/covers
etc. shipper agrees to bear responsibility of damage from such action and leave USA Hauler and its carriers/sub-contractors
harmless of any and all damage as a result of loose or improperly mounted tarps,covers,hardware etc.
